Advanced Standing Credit Evaluation – For vocational students that may acquire competencies and skills similar or identical to courses offered by the College               

 Application Agreement Statement – If you did not sign your application, did not send in your signature page after completing an online application, or did not pay your application fee online.

Credit by Examination  – Petition to test out of a course(s)

Credit by Examination for CPT 104/CPT106  – Petition to test out of CPT104 or CPT106

Credit for Experience  – Petition to receive credit for a course based on previous experience in the subject

Credit for Non-Academic Learning  – Petition to receive credit for a course based on training received from a source other than an institution of higher learning

Dental Hygiene Observation  – Must be completed by Dental Hygiene students before starting clinicals.

Human Services Questionnaire  – Must be completed for formal application to Human Services

Major Revision  – To change a major or add a second major

Nursing Information Sheet  – Must be completed by Nursing students before starting the Nursing Program

Nursing Legal Form  – Must be completed by Nursing students before starting the Nursing Program

Nursing Program Technical Standards – Must be completed by Nursing students before starting the Nursing Program

OTA Observation  – Must be completed by Occupational Therapy students as part of the qualification process

Petition for Reinstatement  – Petition for readmission after being academically dismissed from Rhodes State

Physical Therapist Assistant Legal Form  – Must be completed by Physical Therapy students as part of the qualification process

Physical Therapy Observation   – Must be completed by Physical Therapy students as part of the qualification process

Program Change with Grade Point Adjustment  – For students planning to change their program to something completely different than their current program to improve their GPA

Reactivation  – For a previous student that attended Rhodes State or Lima Technical College and received grades, and has been 3 or more consecutive quarters since attendance

Residency - Must be completed by students with out-of-state status

Respiratory Care Observation  – Must be complete by Respiratory Care students before starting the Respiratory Care program

Statement of Draft Registration  – All males between the ages of 18 and 26 must provide this number to the Admissions and Advising office if the information was not previously on the application for admission
